　　In accordance with Article 15 of the Enforcement Regulations of the Johkasou Act, the following documents must be attached to the application form:

(1)A copy or extract of family register or a statement of registered domicileResident cardOr a document in lieu of these

　　　(Certification must be dated within the last 6 months of the application date) *Copies are not acceptable.

　　　(The documents that can be substituted for these are the All Particulars Certificate or the Individual Particulars Certificate, etc.)

(2) Johkasou Operator ExamCopy of the certificateOr Johkasou Operator TrainingCopy of certificate

　　　Or a copy of your driver's license(Except for cases where the maiden name is listed on the residence certificate in (1) above.)

　　　*To have your maiden name listed in the former name column, you must apply to your city or town.

(4)In case of a rewrite application, the Johkasou Operator license that has been issued

　*　Applications for Johkasou Operator licenses can also be submitted via email.

　　In that case, please attach the various application forms (PDF) to an email and send it to the address below. The subject line of the email should be:

　　For new applications, enclose "Johkasou Operator License Application Form Enclosed". For reissuance applications, enclose "Johkasou Operator License".

　Please clearly mark the envelope with "Application for Reissuance Enclosed," or "Application for Renewal of Johkasou Operator License Enclosed" if applying for renewal.
